Technical surgical options for sphincter preservation for the very low rectal cancer or the rectum destroyed by disease or surgery are presented.  The authors demonstrate three approaches to direct anal or supra-anal anastomosis:  abdominal; combined abdominal transsacral; and transanal abdominal transanal.
